FATAL FURY 3 ACA NEOGEO retains the gameplay and controls of the original game, with the addition of a third plane in the Oversway System. Players can strategically use the additional planes in the background (LP+LK) or foreground (LK+HP) to gain an advantage during combat.

After executing an offensive move in a tilted or off-balanced position like the Oversway Attack, the player's character will reposition back to the standard orientation. To counter an opponent who is in a tilted position, the player can use an Anti-Oversway Attack. Alternatively, the player can evade an enemy's attack by quickly shifting to the side (Quick Sway), leaving the opponent vulnerable to a conventional strike. By adjusting the character's jumping height, utilizing a defensive move while airborne (Air Guard), and employing a new combo technique known as Combination Arts, the player can interrupt a specific sequence of regular attacks (which differs depending on the character) with consecutive strikes.

Every character in the game has regular Special Moves and Super Special Moves that are only available when their life gauge is flashing red. Additionally, each character possesses a "Hidden Ability," which is a more powerful version of a Super Special Move that can be utilized in extreme situations. This Hidden Ability will be triggered every 1024 times the player inputs the command for the character's Super Special Move. Players can also activate a character's Hidden Ability and enter "Super Mode" at the start of a game by using a secret code. To activate the Hidden Ability when a player's life bar is in the red, they must input a specific command. Unlike Super Special Moves, a character's Hidden Ability can only be used once per round.

The player has the option to select their initial adversary from a selection of four opponents (Joe, Mary, Bob, and Franco). After conquering the first four opponents, the player will face Ryuji Yamazaki in a one-round storyline-driven battle. The player must defeat Mai, Andy, Hon-Fu, Sokaku, Terry, and Geese before encountering Yamazaki in a complete match. Depending on how well they perform throughout the game, the player will eventually face either Yamazaki or the Jin twins, Chon Shu followed by Chon Rei.
